[underscore] 3 | [blank] | [blank] | at $[blank]/100 per cord, DOLLARS. [underscore] [blank] CTS. [underscore] [blank] I certify, on honor, that there were no quarters owned or hired by the public, at the above station, which could be assigned to {Act. Asst Surg L.C. Crump} during the above period; and that the fuel is charged at the average market price for the month. [signature illegible] Quartermaster. I certify, on honor, that the above account is correct and just; that I have been regularly stationed on duty at {Chimborazo Hosp} by {Med. Dir.} during the period charged for; that I have not been furnished with quarters, rent, or fuel, by the public, nor received a commutation of money in lieu thereof.
